Group Accident Insurance Summary

Client: Off road vehicle clients of Prestige Insurance Brokers a/o Outdoor Adventure & Sport Insurance Solutions

Insured's:

1. All off road vehicle owners, owners family and occupants of vehicles driven by the owner or family member who is insured through the "Open Skies" off road vehicle program.

Coverage: Bodily Injury caused by an accident occurring while using or operating the vehicle insured under the clients off road liability package within the territory of Canada and The United States of America. Subject to standard exclusions including exclusions; while used for illicit or prohibited trade or transport, participating in race or speed contest or practice or while driving when not authorized or qualified to operate the vehicle.

Benefits:

Death
a. under age 5 = $500
b. 5-9 years old = $1,000
c. 10 and over
   i) $5,000 if Head of household
   ii) $2,500 if spouse
   iii) $1,500 if dependent child

Supplemental Death Benefits
Lump Sum - For Adults an additional $1,000 if survived by a spouse and dependent child or 2 or more dependent children
Weekly Payments - conditional on there being a survivor, $145 for 1st survivor plus $35 for each additional survivor. Maximum payment of 104 weeks, terminates earliest of 104 weeks payment or death of all remaining survivors.
Funeral Expense - $2,500

Disability
a. employed insured - $300/week for total disability, payable from the 8th day for up to 104 weeks, maximum age 65 and subject to 75% all source.
b. homemaker - $145/week for disability that prevents them from performing household duties, payable from the 8th day for up to 104 weeks, maximum age 65

Medical

Maximum of $25,000 per injury or sickness within a 52 week period. Includes expenses for;

Ambulance, private duty nursing, dental injury, physiotherapy(max 12 visits), chiropractor, prosthesis, durable medical equipment , drugs, other misc expenses such as hearing aids, crutches.

Aggregate Limit: $500,000 Any One Accident
